Below please find my Health & Wellbeing Form required to be completed when receiving an  Integrative Healing Session. When filling out my intake form you have two options.

1.   You can download the form, print it, fill it out and bring it with you on your initial visit.  

2.  You can download the form, fill it out online and email it back to me prior to your appointment.
